Home Studies
The Miriam Project is licensed in the state of Indiana to provide Home Study services for both domestic and international adoptions.
A Home Study involves visits to your home where your environment and family may be observed, as well as collecting and completing various documents. To begin the process, please turn in an application with your application fee and the Data Collection Form. Once we have received your application, we will contact you to schedule your first home visit. We recommend that you begin the process of completing the remaining forms and collecting the necessary documents. Please contact us with any questions.

- One application for The Miriam Project.
- Data Collection Form
- One Certified Copy Birth Certificate per parent
- One copy of the Marriage Certificate
- One copy of a Divorce Decree (if applicable)
- One Miriam Project Financial form* a. Supporting Documents for Financial Profile: 2 consecutive pay stubs per adult, 2 consecutive bank statements for savings & checking, most recent 401K report, any other documents to support information on financial profile such as stocks, bonds, etc.
- One Medical Report For Primary Caregiver per parent*.
- One Medical Report For Household Members per each other person in the home*.
- Child Protection Service reference checks for each adult in the home (completed by the Miriam Project)*.
- Four non-family references with one being from your minister or pastor of faith.
- Statement of medical insurance coverage for adopted child.
- Statement of explanation about any children no longer living in the home.
- Release of Confidential Information as needed.
- Federal/State Criminal History check per adult in home (fingerprinting completed at your local Department of Child Services branch)*.
- Local Police Check per adult in home*.
- Checklist for Special Needs Child.
- Checklist for Domestic Transracial/Transcutural Child.
- Complete Education requirements.
- 2 originals family and individual photos with letter to birth family - completed for presentation to the birth family. (Domestic Adoption Only)
- Guardianship Statement (International Adoption Only)
- Verification of Residence (International Adoption Only)
- Certification of Assets (International Adoption Only)
- Additional information may be requested.
* A Home Study expires after twelve months and/or after a placement is completed. Following the twelve months an update is needed; these documents are needed to complete the Home Study Update.
