Application Documents
The following documents need to be completed to begin the adoption process with the Miriam Project. You will need Adobe Acrobat Reader to view and print these PDF documents.

Required Documentation
These documents required to begin the adoption and home study process with the Miriam Project. When completed, please return all documents to:

- Application Form and Data Form

Remember to sign the Statement of Faith at the end of the application. Please submit the application fee of $175.00 (payable to: Miriam Project) along with your application.
- Medical Report For Primary Caregivers
One for the husband and one for the wife. Take it to your physician for completion. Upon completion, return them to the above address with the application form. They must be completed within 6 months of the initial application.
- Medical Report For Household Members
One for each adult or child living in the home aside from the primary caregivers. Take it to your physician for completion. Upon completion, return them to the above address form. They must be completed within 6 months of the initial application.
- State-Wide Criminal History
Please review and submit the required request form to the address provided.
- Local Police Check
Complete and return to the Miriam Project.
- Financial Profile
Complete and return with the application form.
- References
Four non-family references, with one being from your minister/pastor from your faith community. Complete the top part and forward to the each referee with a stamped envelope addressed to the Miriam Project.
